Registered Nurse for Hospice Care

Role:
MDS Nurse responsible for managing the Minimum Data Set (MDS) process, care planning, and coordinating assessments in compliance with regulatory standards.
Key Responsibilities:

Lead and ensure high-quality, person-centered care
Develop, complete, and transmit assessments
Review and manage reimbursement
Case manage care plans, including rehabilitative programs
Collaborate with families regarding resident care
Qualifications & Skills:
Current OH RN license; BSN preferred; at least 3 years' MDS experience in long-term or post-acute care; strong knowledge of clinical standards, regulations, and reimbursement; Ability to recognize resident dignity, communicate with care, and make accurate judgments.
