Schedule
- Shift: 8-hour days only
Assignment Details

• Patient load: 1:1 ratio; may visit up to 6 patients/day
• 30 productivity points standard

Requirements

• Minimum: 1 year skilled home health RN experience required
• Oasis experience: Required
• BCLS (Basic Cardiac Life Support) certification required
• Licensure: OR & WA RN licensure required (pending licenses accepted)
• Key clinical skills: wound care (including wound VAC), disease management, PICC lines, G-tubes, ostomy care/education, catheter care

Roles & Responsibilities

• Conduct patient assessments and develop care plans
• Administer medications and treatments as prescribed
• Provide wound care, including management of complex wounds
• Educate patients and families on disease management and care procedures
• Ensure compliance with documentation and reporting requirements

Compliance / Notes

• Orientation: ~40 hours
• Staff expected to have personal transportation; system groups clients to minimize drive time
• Confirm vaccination requirements with hiring manager
