Infusion Nurse I A licensed registered nurse who provides basic direct patient care to patients in the home or an alternate infusion site, ensuring patient safety and continuity under a physician’s plan of care. Works under close direct supervision of the designated supervisor.
Job Responsibilities Initiate, develop, and implement basic nursing plans of care treatments, evaluating patient progress toward goals.
Participate in direct patient care, perform treatments, administer medications, and educate patients and families/caregivers.
Modify plans of treatment in response to changing patient status based on physician orders to achieve established or revised patient care goals.
Assess patient needs and physical status at each visit through health data access and patient interview.
Re‑evaluate patient needs through physical reassessment, therapy response, and supplemental physician orders.
Obtain and clarify physician orders for plan of treatment revisions, inform physician promptly of significant changes in patient’s condition, and provide a written summary to physician within supplemental order.
Provide effective and safe teaching using a patient‑centered care approach to achieve independence with prescribed therapy and care needs.
Effectively and timely communicate with the Clinical team, medical providers, patients, and caregivers to facilitate continuity of care.
Prepare clinical documentation in real‑time during visits and submit to the nursing department to comply with established timelines for billing optimization.
Coordinate discharge planning, prepare discharge summaries with patient instructions, and report patient care needs, progress, and goals when transferring care.
Demonstrate compliance with agency operations, policies, professional standards, and regulations.
Maximize work efficiency with computers and automation technologies to validate plans of treatment orders, communicate patient care provided, and follow assigned visit schedule.
Respond promptly to patient requests and initiate emergency procedures as necessary.
Accept accountability for own practice through ethical and professional conduct.
Maintain competency and licensure through required training, in‑services, and continuing education.
Provide oversight and coordination of paraprofessionals in the home setting, and act as Supervisor in the absence of the Supervisor or Nurse Manager as applicable.
Participate in multidisciplinary team conferences and precept, train, and mentor other nurses for orientation, onboarding, and supervisory activities.
Participate in nursing on‑call responsibilities as applicable.
Perform other duties as directed by supervisor.

Education and Experience Requirements Active and unrestricted RN license in the state of practice.
Minimum 0‑2 years of infusion nursing experience.
Current CPR certification (in‑person class required).
Qualifications Demonstrated competency in patient care standards and infusion skill sets.
Advanced certification and training as applicable.
Valid driver’s license in state of practice with access to a vehicle for business travel and proof of liability insurance.
Basic knowledge of computer operating systems and software applications.
Physical Demand Requirements Ability to lift up to 50 pounds, with a maximum lifting of 75 pounds. May require walking, standing, kneeling, turning, pivoting, balancing, stooping, reaching overhead, pushing, pulling, carrying. Fine motor skills and visual acuity required.
Potential exposure to blood, body fluids, hazardous substances, and unpredictable home environments. Safety requirements include closed‑toe shoes, facemask, goggles, gown and/or gloves, and functioning communication device.
Travel Requirements Willing to travel 100% of the time for business purposes.
Preferred Qualifications & Interests Bachelor of Science in Nursing (preferred).
One year of prior RN experience in a home health or acute care setting.
