The Licensed Practical Nurse (LPN) works under the direction of a primary care provider, as part of the healthcare team, and is responsible for providing general clinical nursing support. The individual is accountable for assessing, planning, implementing and evaluating the nursing care required for the patient and will assist in the development and revision of patient’s care plan. Prepares instruments, equipment, materials and exam rooms to ensure the appropriate items are available for patient care. Obtains and documents complete patient data in a timely and accurate manner to ensure the accuracy of the patient record for future use. Assists the provider with exams, tests and procedures and provides patient instructions to provide appropriate patient care and ensures the patient is knowledgeable on self‑care procedures. Completes clerical or insurance processes to ensure the proper documentation and submission of charges is completed in a timely manner. Participates in the development of the care plan with other team members to safely and accurately implement the plan. Completes steps to fill prescriptions per protocol and completes prior authorization forms to ensure proper billing and fill of the prescription for the patient. Graduation from an accredited Licensed Practical Nurse (LPN) program.

Experience as a licensed practical nurse within an area relative to the hiring department preferred.

Licensed Practical Nurse license awarded by the State of Michigan.

Wisconsin LPN license must be obtained within the first 3 months.
