RN/LPN Full-Time Care Center - NOC Shift

Job Summary Responsible for enhancing and improving the SRIM experience specifically in heartful care for our residents and their families. Demonstrates and promotes SRIM’s core value of the golden rule by treating residents, families, visitors and associates in the same way we would like to be treated. Performs assigned nursing procedures for the comfort and well‑being of residents such as assisting in admission of new residents, assisting with activities of daily living when needed, administering specified medications, supervising non‑licensed nursing staff and participating in their evaluation.
Responsibilities Demonstrates excellent decision‑making skills and can understand the impact of those decisions.
Sets and manages priorities.
Accepts calls from staff during nights and weekends.
Meets with oncoming and departing licensed practical nurses to discuss resident’s condition and any occurrences from the shift.
Responsible for making sure communications are clear, accurate and timely.
Counts controlled substances with outgoing nurse, assumes responsibility for assuring accuracy of count before accepting keys.
Checks, orders, and administers prescribed medication with 100% accuracy.
Ensures the correct dosage by never pre‑pouring medications.
Demonstrates knowledge of important medication interactions and side effects.
Ensures medication cart is locked at all times.
Obtains pertinent information and completes all required forms as established by policy and procedure upon resident admission.
Responsible for completing and maintaining the ECC and LNS logs, service plan, progress notes, monthly summaries and following healthcare providers’ orders.
Assists in completing evaluations when needed for residents.
Provides hands‑on care on an as‑needed basis to meet the needs of the residents.
Works with the ADORS in reviewing and investigating all incident reports and recommends appropriate action and follow‑up to the DORS/executive director.
Understands and adheres to the ALF regulations pertaining to assisted living health care laws.
Maintains a safe and secure working environment free of objects/situations that could cause harm and/or falls to residents, families, visitors and associates.
Responsible for practicing good body mechanics and safe working habits.
Knows, understands, and follows established policy and procedures in emergency situations.
Takes time to give associates in nursing daily feedback on their performance.
Participates in associates’ performance evaluations when requested by the DORS.
Ensures associates participate in activities provided to residents as described in the activities calendar.
Responds promptly and courteously to residents’ requests for assistance, including phone calls and requests from family and visitors.
Ensures there is a safe, clean and orderly environment for all residents.
Ensures a dignified and caring atmosphere for residents, families, visitors and associates.
Wears clean and appropriate dress per established policy.
Complies with HIPAA regulations.
Considers themselves essential personnel especially during emergencies.
Takes initiative in work, seeks out additional information and offers solutions/suggestions.
Performs all other reasonable duties as assigned or requested.
Willingly takes on special projects or assignments outside of regular duties.
Supervisory Responsibilities Directly supervises employees in the Resident Services/Nursing Department. Carries out supervisory responsibilities in accordance with the organization’s policies and applicable laws. Responsibilities include training employees; planning, assigning, and directing work; appraising performance; rewarding and disciplining employees; addressing complaints and resolving problems.

While performing the duties of this job, the employee is regularly required to stand, walk, use hands, reach with hands and arms, and talk or hear. The employee is frequently required to stoop, kneel, crouch or crawl. The employee is occasionally required to sit, climb or balance, and taste or smell. The employee must occasionally lift and/or move up to 50 pounds.

Qualifications Language Ability: Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations. Ability to write reports, business correspondence, and procedure manuals. Ability to effectively present information and respond to questions from groups of managers, clients, customers, and the general public.
Math Ability: Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals. Ability to compute rate, ratio, and percent and to draw and interpret bar graphs.
Reasoning Ability: Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists. Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form.
Computer Skills: Ability to use word processing software and electronic medical records.
Education/Experience: High school diploma or general education degree (GED); and one to two years related experience and/or training; or equivalent combination of education and experience.
Certificates and Licenses: Valid state‑specific nursing license.
