How you’ll make an impact in this role Assess patients with chronic or acute heart failure through physical exams, symptom review, health history evaluation and monitor vital signs and weight trends. Educate patients and families on heart failure management, medication adherence, and lifestyle changes Coordinate care plans with providers, including titrating medications per protocols. Manage incoming calls to assess symptoms, adjust care or refer to a physician. Act as a liaison between physicians and patients, facilitating follow-up appointments and diagnostic tests. What minimum requirements you’ll need Licensure / Certification / Registration: BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date required. Licensed Registered Nurse credentialed from the Tennessee Board of Nursing OR current home state license for multi-state license recognition "Compact State" obtained prior to hire date or job transfer date. Education: Diploma from an accredited school/college of nursing OR Required professional licensure at time of hire.
