Information Contact and Assessment Service; ICAS, is a front door social care team where workers complete assessments, initial safeguarding concerns, mental capacity assessments, preventative assessments, and assess and review low level equipment provision. We need to be knowledgeable about the range of teams within adult social care, their remit and referral routes. We need to be aware of current community resources to support our preventative offer and to work co-operatively and collaboratively with our colleagues in AgeUK Lambeth.

You'll need to adapt to a demanding and changing environment and have a commitment to partnership and collaborative working.

The team support hybrid working, the requirement is to be on site 3 days a week with the option to work 2 days at home.

9 Care Act Assessments with adults over 18 years of age, with a range of physical disabilities and health vulnerabilities, so experience with these client groups is essential.

The primary feature of work undertaken by staff is to deliver strength based, person centred preventative support and to complete robust care planning for eligible people.

The Social Worker will participate in our duty rota where we screen both routine and priority referrals and complete urgent home visits

The Social Worker will need some knowledge of Occupational Therapy equipment and minor adaptations, be a Trusted Assessor, or happy to undertake this training

The Social Worker will make effective use of resources through care planning, signposting to other services, or providing individuals with information and advice.

The Social Worker will identify and provide a timely response to safeguarding issues, evidencing your robust understanding of safeguarding procedures.

The Social Worker will have the ability to work at a pace, and safely manage cases to achieve positive outcomes for our residents
