Live Well Homecare in Illinois is seeking a Registered Nurse to provide safe, compassionate care across patient populations by assessing, planning, implementing, and evaluating plans of care.
You will conduct assessments, develop and document care plans, collaborate with healthcare teams, and support admissions and transfers while ensuring effective handoffs.
A current IL RN license, CPR, and CPI training within 90 days are required; psychiatric care experience is preferred.
