Position Overview: The Wellness Nurse (Licensed Practical Nurse) plays a crucial role in supporting the Resident Services Director with the effective implementation, delivery, and coordination of Resident Care Services within the community.
Oversee the Resident Services Department staff, including Certified Nursing Assistants (C.Aid in managing the medication program and assist residents with self-medication.
Foster positive relationships with residents, their families, and healthcare providers.
Report any significant incidents or changes in residents' needs to the Executive Director or Resident Services Director.
Assist in completing resident assessments and developing service plans.
Collaborate with the Resident Services Director in hiring and managing departmental performance.
Participate in training and orientation for new staff members.
Attend scheduled in-service training sessions and team meetings.
Help with the scheduling and staffing of Resident Services Assistants as necessary.
Provide coaching and mentorship to associates in the Resident Services Department.
Support residents and their families to ensure a high level of satisfaction.
Assist in supervising RSA staff during care provision.
Adhere to emergency protocols.
Maintain positive working relationships with residents, families, peers, and other staff members.
Current Licensed Practical Nurse license in the relevant state(s).
Certification in basic first aid and CPR.
Minimum of two years of full-time or equivalent clinical experience.
Preferred experience in assisted living or long-term care settings.
Must be in good physical and emotional health, free from communicable diseases.
Proficient in transferring residents and using a Hoyer Lift and other medical equipment when necessary.
Availability to work weekends and holidays as required.
May need to work shifts different from the one assigned at hiring.
Exposure to infectious waste, diseases, and conditions may occur.
May encounter residents who are hostile or emotionally distressed due to their mental state.
