At Valley Presbyterian Hospital, pharmacists are essential members of the clinical care team, helping ensure safe, effective, and evidence-based medication therapy for every patient. We are seeking a dedicated and clinically skilled Pharmacist to support patient care through medication management, interdisciplinary collaboration, and pharmaceutical expertise.

Key Responsibilities

Review and verify medication orders to ensure accuracy, appropriateness, and patient safety

Monitor and evaluate drug therapies, including dosing, interactions, contraindications, and therapeutic effectiveness

Collaborate with physicians and healthcare professionals to optimize medication treatment plans

Assess patient medication profiles and interpret laboratory results to support clinical decision-making

Educate patients, caregivers, and healthcare staff on medication use and therapy management

Oversee pharmacy technicians and support personnel to ensure workflow efficiency and compliance

Prepare and dispense medications and IV therapies accurately and in a timely manner

Identify, document, and report adverse drug reactions, medication variances, and safety concerns

Assist with formulary management and procurement of non-formulary medications

Participate in quality improvement initiatives, regulatory compliance, and patient safety programs

Qualifications

Doctor of Pharmacy (PharmD) degree required

Active California Pharmacist license in good standing

Ability to obtain LA City Fire Card within 30 days of hire

Preferred Qualifications

Completion of an ASHP-accredited residency program or equivalent clinical experience

Experience with Meditech or comparable pharmacy information systems
