Role Summary As a Wellness Nurse (LPN/LVN), you support resident health, monitor care needs, and ensure safe medication practices and clinical compliance.

Responsibilities Conduct wellness assessments and monitor resident health changes

Communicate with families, physicians and care teams

Monitor incidents, falls and follow-up care

Support infection control and clinical quality initiatives

Train and support care team members
