Join Our Compassionate Team as a Part-time Hospice Registered Nurse Case Manager!

Part-time Hourly Range: $37 - $38 per hour DOE

As a Hospice RN Case Manager , you’ll do more than provide care — you’ll bring peace, comfort, and dignity to patients and families during life’s most sacred journey.

Deliver skilled, compassionate nursing care in patients’ homes and facilities.

Conduct comprehensive assessments and develop individualized care plans that honor patient wishes.

Coordinate care with the interdisciplinary team to ensure seamless communication and holistic support.

Participate in on-call rotations to ensure patients always receive timely, dignified care.

Active RN license (required).

Experience in hospice, palliative, geriatric, or home health care (preferred).

Ability to work independently while collaborating with a dedicated care team.
