Registered Nurse (RN), Home Health - Merced

We are hiring Home Health RNs for Fresno and Merced territories, providing compassionate in-home nursing care supported by experienced leadership and strong clinical resources.

Perform comprehensive in-home nursing assessments for home health patients

Develop and manage individualized plans of care in collaboration with physicians, caregivers, and the interdisciplinary care team

Provide skilled nursing services including medication management, wound care, disease management, and patient education

Identify the need for additional home health services, disciplines, and durable medical equipment (DME)

Obtain and document physician orders, including verbal orders, per agency and regulatory guidelines

Educate patients and caregivers on care plans, medications, and symptom management

Maintain compliance with CMS, Medicare, and home health regulations

Serve as a preceptor or mentor to other nursing staff as needed

You enjoy one-on-one patient care

You take pride in delivering skilled, compassionate care

Graduate of an accredited nursing program

Current, active California Registered Nurse (RN) license and CPR certification

At least one year of clinical practical nursing experience outside of school or rotations

Home Health experience preferred, but not required

Valid driver's license, reliable vehicle, and current auto insurance

Ability to make home visits and provide skilled nursing assessments and care

Home Health and Home Care Home Base (HCHB) documentation experience preferred, but not required
