Key Duties

Deliver home healthcare services in accordance with physician orders and care plans

Maintain accurate electronic documentation using provided tablets

Collaborate with team members and provide patient education

Ensure safety and comfort of patients in their homes

Qualifications & Skills

Active, unencumbered LPN license

Current CPR certification (training assistance available)

TB skin test within the last 12 months

Reliable transportation

Preferred but not required: six months hands-on nursing experience
