We are seeking a dedicated and compassionate Full Time Home Health Physical Therapist (PT) to join our dynamic healthcare team. In this role, you will play a crucial part in enhancing the quality of life for our patients by providing tailored physical therapy services in the comfort of their homes.

Key Responsibilities:

Conduct comprehensive assessments to determine the physical therapy needs of each patient.

Develop personalized treatment plans aimed at improving mobility, strength, and overall physical function.

Implement therapeutic exercises and techniques to address patient-specific goals.

Educate patients and their families on home exercise programs and techniques to ensure continuity of care.

Collaborate with other healthcare professionals to provide holistic and coordinated care.

Document patient progress and modify treatment plans as necessary to achieve optimal outcomes.

Qualifications

Valid state licensure as a Physical Therapist.

Proven experience in home health or a related healthcare setting is preferred.

Strong interpersonal and communication skills.

Ability to work independently and manage a flexible schedule.
