Advanced Licensed Practical Nurse-Primary Care

Position Overview Licensed Practical Nurse (LPN) provides skilled nursing care tasks and procedures under the direction of an RN, nurse practitioner, clinical nurse specialist, physician, or another authorized health‑care provider. The role is based in the UHS Primary Care Walton unit, with a regular day shift of 40 hours per week.
Compensation Hourly rate $22.97 - $34.46, depending on experience.
Responsibilities Assist RN or other authorized provider with patient care services.
Administer medications and treatments as prescribed.
Monitor and document patient vital signs and clinical data.
Collaborate with interdisciplinary team to support patient recovery.
Maintain accurate and timely patient records.
Education & Experience Graduated from an accredited school of Practical Nursing.
Current New York State Practical Nurse License, a Limited Permit, or eligibility to practice within the 90‑day grace period per state guidelines.
Preferred: Basic Skills Certificate (CPR).
