Job Description Horizon Healthcare, Inc. & American Telehealthcare, Inc. haveimmediate openings for full-time and part-time Nurse Practitioners and Physician Assistants. Working in consultation with our physicians (psychiatrists and addictionologists) and other healthcare staff, the incumbent's primary responsibility will be medication management and behavioral health assessments. Specific duties will include case management, medication prescribing, and patient education and coordination of care in on outpatient setting. Services will be provided via telehealth.

Services we provide include: psychiatric assessments, psychotherapy, and medication management all via a secure, HIPAA-compliant cloud-based network. The platform integrates and synchronizes in real time high definition video-conferencing, electronic medical records, scheduling, electronic prescription of controlled substances, and billing. REQUIREMENTS:

• Valid Nurse Practitionerlicense in the State of Wisconsin (or willing to apply)
• MSNCertificationfrom an accredited university
• Board Certification in Family Practice, Adult Practice, Geriatrics, Primary Care, or any Psychiatric field
• Willing to commit at least 8 hours per week to providing care to our patients
• Liability insurance in at least the minimumamount required by WI law If you have experience with telehealth great! If not, we can provide all the equipment necessary as well as the necessary trainingto help make you an expert in delivering care via telehealth.
