Home Health Care Physical Therapist (PT) - Full-time

Vitality Care Clinic is seeking a dedicated and skilled Physical Therapist (PT) to join our team in Huntington Beach, California. As a Physical Therapist at Vitality Care Clinic, you will operate predominantly in a home care setting, providing personalized treatment in the comfort of patients' homes. You will be part of a cohesive team that values collaboration, innovation, and a patient-first approach.

Conduct comprehensive Physical Therapy evaluations for pediatric clients, assessing their physical abilities and needs.

Design and implement individualized treatment plans that align with the patients' goals and therapy objectives, ensuring the highest standard of care.

Provide education and support to patients and their families about the recovery journey, potential challenges, and the importance of adherence to therapy protocols.

A Bachelor's degree in Physical Therapy from an accredited program is required; a Master's degree or Doctor of Physical Therapy (DPT) degree is strongly preferred.

Possession of an active, unrestricted license to practice physical therapy in California is mandatory.

Willingness to provide therapy services both in person and via telehealth, as appropriate for client needs.

Ability to work flexibly in a rapidly changing healthcare environment.

Exceptional collaboration skills, demonstrating a commitment to team-based care and superior customer service.

Experience in home health therapy or pediatric physical therapy is highly advantageous.

Proficiency in utilizing electronic health records systems for patient documentation.

Familiarity with multidisciplinary collaboration in therapy services.
