The ideal candidate is a skilled professional that is passionate about delivering quality care and making a real difference with 1 year of experience in a Home Health setting.
Responsibilities
Assess each patient and develop an appropriate treatment plan with measurable goals and time frames
Prepare and submit clinical and progress summaries based on identified and attained goals
Ensure that other treating medical professionals have the necessary information to appropriately evaluate the patient
Experience with Oasis and Homecare Homebase a plus
Pay is $34.50 an hour with a nontax weekly stipend of $1631 per week
Qualifications
Bachelor\'s, Master\'s or Doctor\'s degree of an accredited physical therapy curriculum
Certification in Physical Therapy and valid CA state license to practice
