Remedy Home Health Care is actively seeking a licensed Physical Therapist (PT) to join our growing Home Health team in Orange County. This Per Diem Physical Therapist role offers flexibility, autonomy, and the opportunity to provide high-quality rehab and community-based care directly to patients in their homes.
As a Home Health Physical Therapist , you will work with senior care populations , including geriatric patients recovering from illness, injury, or surgery. You’ll deliver skilled therapy services in home-based, assisted living, and community care settings , helping patients regain independence and improve their quality of life.

Your Role: Home Health Physical Therapist (PT)
As a Physical Therapist in Home Health , you will provide skilled rehab services tailored to each patient’s needs. Your focus will include fall prevention, strength building, mobility training, and functional independence for senior and geriatric patients .
Conduct in-home physical therapy evaluations and reassessments
Develop and implement personalized rehabilitation care plans
Provide therapy in home health, assisted living, and community care settings
Document visits using WellSky (Kinnser) EMR
Collaborate with interdisciplinary home health teams
Licensed Physical Therapist (PT) – California (Required)
Minimum 1 year of clinical experience (Home Health preferred)
Experience in senior care, geriatrics, rehab, or community-based care preferred
Valid driver’s license, reliable transportation, and auto insurance
Flexible Per Diem / PRN, Part-Time, or Full-Time opportunities
Competitive per-visit pay + mileage reimbursement
Hands‑on experience in home health and rehabilitation care
Work with geriatric and senior care populations
Independence in a mobile, community-focused role
