Bachelor’s degree in the field of Social Work/Psychology/Gerontology or related field with strong writing and verbal communication skills

A minimum of two years of experience is required working with geriatric residents in Long Term Care setting

Knowledge of Title 22 Regulations and case management

Ability to administer Depression Scale and Cognitive Testing

Residential Social Services shall plan and coordinate social services for the residents needing support in Independent, Assisted Living, and Memory Care units; establish and facilitate a comprehensive support (group and individual) program for all residents with appropriate follow up.

Develops and implements a case management program for providing psychosocial support to all residents

Assist residents in transition between the various levels of care as necessary and provides support for families to help them deal with these transitions.

Provides input for departmental operation and capital budgets, including possible revenue generated from counseling programs and case management.

Consults with the Director of Resident Services, Director of Residential Continuing Care Director of Activities, and Administration to develop a holistic program to meet the needs of residents, on their individual levels, to enhance the quality of life.

Coordinates regular, ongoing educational sessions for residents on age-related topics.

Assumes a proactive role in resolving resident issues & concerns related to assigned areas of responsibility. Includes Medicare, and Long-Term Care Insurance. Available to meet and support residents with various requests and any other duties as assigned.

Provides input to RHAT (Resident Health Assessment Team) through personal observations and those of staff regarding changes in resident condition and develop an action plan.

Assesses residents’ cognitive, functional, and psychosocial abilities on an annual basis.

Serves as a liaison for residents in skilled nursing.

Monday, Wednesday and Friday 8:30am - 5:00pm
