Licensed Practical Nurse - Night Shift - Health Care

Job Description Summary
UofL Health is hiring Licensed Practical Nurses (LPNs) for PCU, psych, and rehab units throughout our system. Positions are available on a part‑time and full‑time basis, with days and nights scheduled. Locations include Frazier Rehab, Jewish Hospital, Mary and Elizabeth Hospital, Peace Hospital, UofL Hospital, and UofL Health. UofL Health is a not‑for‑profit 501(c)(3) academic health system dedicated to transforming community health through compassionate, innovative, patient‑centered care.

Position Summary and Purpose
The LPN provides patient care using the nursing process. The nurse collaborates with the interdisciplinary team and ancillary staff to deliver safe, quality care and promote patient recovery and functional improvement.

Essential Functions

Demonstrates knowledge of nursing theory and practice within the scope of practice defined for LPNs by the Kentucky Board of Nursing.

Completes a comprehensive daily assessment on assigned patients, documented in the patient’s record.

Prioritizes care based on assessment of patient needs, evidenced by patient progress toward established goals.

Reviews and revises patient care plans according to policy and procedure.

Delivers direct patient care based on physicians’ orders and nursing interventions established on the care plan.

Documents interventions and progress toward patient goals.

Monitors laboratory values, notifying the nursing manager and/or physician of any abnormalities within the shift, and ensures critical lab results are reported within 30 minutes.

Administers and documents prescribed medications (scheduled/PRN) according to schedule and procedure.

Initiates nursing care by securing and implementing physicians’ and nursing orders without direct supervisor instructions.

Reviews and ensures implementation of all MD orders each shift.

Assists in implementing appropriate teaching programs for individual patients.

Develops, delivers, and documents patient and/or family education to meet goals.

Orders self‑care instructions and discharge follow‑up care, complete documentation prior to patient discharge.

Communicates effectively with interdisciplinary team members.

Reports patient information/status per policy and procedure to shift nurse and supervisor, ensuring no incidents of failure to follow guidelines.

Communicates changes in patient condition directly with physicians and team members, ensuring timely intervention.

Maintains chart round documentation with minimal omissions.

Provides supportive communication to patient families and personnel, with no documented complaints.

Identifies and escalates unresolved issues to appropriate personnel.

Delivers age‑appropriate care, including medication therapy evaluation for geriatric, adolescent, adult, and pediatric patients as applicable.

Calculates dosage adjustments for geriatric patients and pediatric medication dosing.

Other Functions

Completes special projects within established time frames.

Maintains patient care areas neat and clean per policy.

Labels patient supplies accurately, with no omissions.

Ensures equipment functions properly before and during patient use.

Follows universal precautions and infection control policies.

Plans for patient safety, following safety manual procedures.

Contributes to performance improvement activities to enhance patient outcomes.

Wears name tag and locator badge at all times.

Attends and participates in scheduled shift meetings 80% of the time, reading and initialing minutes.

Shares current nursing literature and practice in clinical situations.

Sets and meets annual individual goals for professional growth.

Maintains quality by completing annual competencies within timelines.

Meets employee health requirements.

Completes mandatory modules and in‑services annually, providing evidence of completion.

Maintains compliance with company policies, procedures, and conduct standards.

Complies with HIPAA privacy and security requirements.

Performs other duties as assigned.

Job Requirements (Education, Experience, Licensure and Certification)

Education: Completion of an LPN program at an accredited school of nursing (required).

For LPNs who graduated prior to September 15, 2019, completion of a Kentucky Board of Nursing approved IV course is required (excluding UofL Physicians and Peace Hospital).

Experience: Prior acute care or rehab experience (preferred).

Licensure: Current license to practice as an LPN in Kentucky (required).

Certification: CPR/BLS required upon hire (American Heart Association).

Job Competency: Knowledge, Skills, and Abilities

Knowledge of treatment of human injuries, diseases, and deformities including symptoms, treatment alternatives, drug properties, interactions, and preventive health‐care measures.

Active listening skills—giving full attention, understanding points, asking questions, and not interrupting.

Ability to select and implement training and instructional methods appropriate for the situation.

Ability to collaborate with nurses to implement interventions and demonstrate achievement of patient goals in documentation.

Ability to appropriately delegate tasks to ancillary personnel according to job descriptions.

Effective time‑management skills, completing routine patient care within shift, including treatments, teaching, and documentation.

Communication: verbal and written, conveying information clearly to appropriate audiences.

Reasoning: use logic to identify strengths and weaknesses of alternative solutions and assist others in developing clinical reasoning skills.

Computer: ability to use the clinical documentation system and communication technology for organization use.
