Position Overview (Certified Nursing Assistant – CNA)

Polaris Rehabilitation & Care Center is seeking compassionate and dependable Certified Nursing Assistants (CNAs) to provide direct, hands-on care to residents in our skilled nursing environment. This role is essential in supporting residents with activities of daily living while promoting dignity, safety, comfort, and overall well-being.

As a CNA, you will work closely with licensed nursing staff and interdisciplinary team members to deliver high-quality, resident-centered care in accordance with individualized care plans, facility policies, and applicable regulatory standards.

Obtain, record, and report resident vital signs, including temperature, pulse, respirations, and blood pressure

Promptly communicate abnormalities or changes in resident condition to licensed nursing staff

Assist residents with mobility, including transfers, ambulation, repositioning, and the use of mobility equipment

Provide support with activities of daily living (ADLs), including bathing, dressing, grooming, toileting, feeding, and personal hygiene

Respond promptly and professionally to call lights and resident requests for assistance

Observe and report changes in resident behavior, mood, mobility, skin condition, appetite, or overall well-being

Accurately document care provided in accordance with facility procedures and regulatory requirements

Maintain a clean, safe, and organized resident environment consistent with infection prevention and safety standards

Follow fall prevention, skin integrity, transfer, and resident safety protocols

Communicate respectfully and effectively with residents, families, visitors, and interdisciplinary team members

Promote resident dignity, privacy, independence, comfort, and quality of life

Adhere to facility policies, resident rights, infection prevention standards, and applicable federal and Wyoming regulations

Current, active Certified Nursing Assistant (CNA) certification in good standing in the State of Wyoming

Previous skilled nursing, rehabilitation, long-term care, or healthcare experience preferred

Ability to adapt and prioritize responsibilities in a fast-paced healthcare environment
