Nurse Practitioner Opportunity

We are looking for a Nurse Practitioner to join our team full-time. Team-Based Care Model: Collaborate with a diverse team focused on whole-person carephysical, emotional, and social.

Enjoy a lower daily patient volume to foster deeper relationships and deliver more personalized care.

You will report to the Associate Medical Director of KY: Dr. You will work on site at CenterWell Regency: 171 W Lowry Ln Suite 180 Lexington, KY 40503.

Deliver comprehensive outpatient care to adult and senior patients.

Maintain accurate medical records and documentation.

Diagnose and manage moderately to complex medical conditions.

Participate in clinical quality improvement programs.

Work with interdisciplinary teams to ensure holistic care.

Contribute to strategic programs and innovations in care delivery.

Master's degree in nursing or completion of a PA program with board certification.

• Must be willing to participate in a quarterly limited on-call coverage by phone.
• Proficiency with electronic health records and digital documentation.
• Specialty training in an Outpatient clinical setting in Primary Care, Family Medicine, Internal Medicine, or Geriatrics.

Experience working with senior populations or in Value-Based Care settings.

Proficiency with electronic health records Athena Health and digital documentation.
