This position is responsible for providing skilled nursing care and services to patients in their place of residence, and may case manage nursing and non-skilled care and services when assigned. All nursing services are performed in compliance with state and local laws/regulations, the Nurse Practice Act, and all other healthcare accreditation standards.
Provide nursing services based on the plan of care
instruct patients on and, as appropriate, prepare medications for their care and safety
Excel at the admissions process, including same-day electronic documentation
Remain flexible to accommodate admissions within time, travel, or geographic constraints
Provide and document skilled care on all visits, meeting agency documentation standards

Valid California Registered Nurse license
A minimum of 2 years of experience in a Home Health or Acute Care nursing setting
Computer skills are a must, including typing skills for reporting and updating documentation in an online portal
able to communicate clearly with staff and the general public, both verbally and in writing
