This role is fully remote, allowing for a flexible work-life balance while offering services from a familiar, comfortable environment. The remote setup enhances accessibility to both clients and resources, ensuring that you can perform your duties effectively while maintaining personal well-being. Consistently provide timely, scheduled appointments for clients, ensuring they receive the attention and care needed at critical junctures in their lives.

Participate actively in a multi-disciplinary team to create and refine an overall treatment strategy, incorporating diverse professional insights into client care.

Monitor ongoing client situations, conducting follow-ups to assess improvements and emotional well-being over time.

Create and implement targeted interventions based on clinical assessments, ensuring alignment with best practices in behavioral health.

Engage collaboratively with family members and school staff when applicable, addressing the specific needs of students within supportive environments.

This role primarily focuses on individuals receiving hospice care, who may be grappling with complex psychosocial challenges related to terminal illness. As an LCSW, you will address the emotional, social, and environmental factors that impact their quality of life. A Master's Degree in Social Work from an accredited program is required, reflecting your advanced understanding of social work principles and ethics.

A valid and active Clinical Social Worker License (LCSW), maintaining compliance with state regulations for practice.

A minimum of 2 years of experience in a behavioral health setting, ideally enhanced by direct experience in hospice or end-of-life care.

Strong case management skills, evidenced by familiarity with various assessment techniques, including suicide assessments and forensic designations.

Knowledge of treatment methodologies specific to hospice care and familiarity with community resources and agencies.

Demonstrated ability to collaborate effectively with healthcare personnel and community organizations to enhance client support.
