We are looking for a clinically experienced nurse who has nursing documentation/charting skills and is a fast learner with great communication skills. The LVN supports the Director of Nursing, Case Management and the nursing staff in the delivery of health care by performing patient care assessments and a variety of medically related tasks. The LVN is a licensed nurse who works in a Home Care Setting to provide nursing services.

At least two‑three days each week (or ten productivity units)

One weekend a month

Two holidays per year (one winter and one summer)

Providing nurse visits to home health patients.

Care for ill, injured, or convalescing patients by implementing plan of care initiated by RN.

Provide basic patient care and treatment. Document consistently with the plan of care.

Educate the patient on how to take care during non‑visit days.

Demonstrate knowledge of regulatory requirements.

Visiting LVN for Home Health: 1 year (Required)

General Nursing: 1 year (Required)

Current CA LVN License (Required)

BLS from the American Heart Association and/or the American Red Cross (Required)NSO Nursing License Insurance (Required)

IV/Phlebotomy Certified (Preferred)

Graduate of an accredited nursing program.

Current, active license as an LVN by the California Board of Vocational Nurse and Psychiatric Technician Examiners.

Current CPR, TB and health clearance and clinical skills self‑assessment.

Minimum of one (1) year LVN nursing experience within the last three years.
