Part Time Caseworker

Social Worker – Part‑time (24 hrs/wk) – Fremont & Grand Haven, MI

DaVita welcomes a Social Worker to join the patient support team in a part‑time capacity. The role focuses on helping patients understand their rights and responsibilities and supporting them through the physical, mental, emotional and financial demands of End‑Stage Renal Disease.

Responsibilities

Advocate for patients and help them understand their rights and responsibilities in the dialysis setting.

Guide patients in managing the physical, mental, emotional, and financial aspects of End‑Stage Renal Disease.

Provide patient‑centered support and maintain long‑term relationships with patients and families in an outpatient setting.

Qualifications

Master's degree in Social Work (MSW) with a specialization in clinical practice.

Valid state licensure to practice in the dialysis setting.

Knowledge of government and private insurance programs.

Basic computer proficiency in MS Word, Excel, PowerPoint, Outlook and DaVita‑specific applications within 60 days of hire.
