Physiatrist- Telemedicine
Azza HealthCare Agency
We are seeking licensed Physical Therapists to provide personalized, one-on-one care for older adults in private homes and senior living communities.
Responsibilities include conducting evaluations, developing individualized treatment plans, delivering evidence-based therapy to enhance mobility and independence, documenting in EMR, collaborating with interdisciplinary teams, and educating patients, caregivers, and staff.
Optional telehealth/virtual care support may be incorporated for increased flexibility and care continuity.
Ideal candidates value autonomy, flexibility, and meaningful patient outcomes, with the ability to create and manage their own schedules.
Qualifications include a valid license, degree from an accredited program, computer proficiency, and CPR certification.
Physical demands involve assisting with transfers, repositioning, walking, climbing stairs, and driving to patient locations.
