As a Family Medicine Physician at Tapestry 360 Health, you will provide comprehensive primary care to patients
of all ages, ensuring their physical and emotional well-being. team, delivering care that spans prevention, diagnosis, treatment, and follow-up, with a focus on addressing
health disparities in our community. You will also play a key role in community health initiatives aimed at
improving overall well-being through outreach and education.
o Comprehensive Care: Provide patient-centered primary care, including exams, diagnostics, treatment,
o Team Collaboration: Collaborate with nurse practitioners, physician assistants, and other health
professionals to ensure seamless, coordinated care.
Provide clinical supervision to nurses and medical assistants, contributing to
the professional development of the healthcare team.
o Community Engagement: Participate in public health initiatives, addressing issues such as immunization,
diabetes management, and hypertension, while ensuring access to care for all community members.
o Documentation & Compliance: Maintain accurate electronic medical records (EMR) and ensure
compliance with healthcare regulations, including privacy standards.
Actively engage in quality improvement activities, including peer reviews and

o Education: MD or DO from an accredited medical school, with Board Certification (or eligibility) in Family
Medicine.
Active Illinois medical license.
o Experience: Previous experience in a community health setting or working with underserved
