Job Description Description: - Document monthly vital signs and weights - following up on significant changes

Complete Incident Reports
Administer Medications when needed
Supervise CMT's and CNA's
Communicate with Care Providers in regards to clients care.
Implement and obtain orders from Physicians
Coordinate supplies with Health Director
Maintain infection control policies and procedures.
Document in charts - Progress Notes etc.

Requirements: Licensed LPN

3 years of experience
