Hospital Liaison

Position Summary
The Hospital Liaison is responsible for developing and maintaining strong relationships with hospitals, physicians, case managers, social workers, acute rehabilitation hospitals, LTACHs, hospice organizations, and other referral partners to increase admissions and occupancy at Warren Nursing & Rehab.

This role serves as the face of Warren Nursing & Rehab in the community and is accountable for generating new referral opportunities, increasing referral conversion, and supporting the facility's specialty programs, including ventilator care and complex medical services.

Essential Responsibilities
Business Development

Build and maintain relationships with hospital case managers, discharge planners, physicians, social workers, LTACHs, inpatient rehabilitation hospitals, and specialty referral sources.

Conduct daily in-person hospital rounds and referral source visits.

Develop new referral relationships throughout the Mahoning Valley and surrounding markets.

Represent Warren Nursing & Rehab at networking events, physician offices, community events, and healthcare organizations.

Identify opportunities to increase referrals from underserved hospitals and physician groups.

Referral Management

Work closely with the Centralized Admissions & Intake team to facilitate timely referral review and admission decisions.

Assist with bedside evaluations and patient/family meetings.

Follow referrals from receipt through admission to improve conversion rates.

Identify and remove barriers preventing admissions.

Provide referral sources with timely communication and clinical updates.

Promote Warren Nursing & Rehab's specialized programs including:

Ventilator & Tracheostomy Care

Respiratory Therapy Services

Pulmonary Rehabilitation

Complex Medical Management

Wound Care

IV Therapy

High-Acuity Skilled Nursing

Short-Term Rehabilitation

Strategic Account Management
Develop targeted growth strategies for:

Acute Care Hospitals

LTACHs

Acute Rehabilitation Hospitals

Inpatient Hospice

Home Health Agencies

Hospice Providers

Physician Practices

Pulmonologists

Cardiologists

Infectious Disease Physicians

Case Management Departments

Performance Expectations

Achieve monthly admission and census goals.

Increase referral volume from assigned hospitals and referral sources.

Improve referral conversion percentages.

Establish new referral accounts each quarter.

Maintain daily activity goals for hospital visits, physician calls, and referral source contacts.

Document all business development activities within the CRM or designated tracking system.

Provide weekly updates on referral trends, competitive activity, and growth opportunities.

Qualifications

Healthcare marketing, admissions, or business development experience preferred.

Skilled Nursing Facility, LTACH, Acute Rehabilitation, Home Health, or Hospital experience strongly preferred.

Strong understanding of post-acute care reimbursement and discharge planning.

Excellent communication, relationship-building, and presentation skills.

Ability to work independently while collaborating with facility leadership and Centralized Admissions.

Preferred Skills

Existing relationships within Northeast Ohio hospitals.

Experience marketing ventilator or high-acuity clinical programs.

Knowledge of Medicare, Managed Care, and Medicaid.

Strong organizational and time-management skills.

Ability to analyze referral trends and develop strategic growth initiatives.
