Job Description Horizon House is a nationally recognized provider of services to adults with support needs due to developmental disabilities, behavioral health issues, and substance abuse. We are seeking qualified Licensed Practical Nurse (LPN ) interested in joining our Residential Group Home team at our Sussex location.

Assist in the development, implementation, evaluation, and revision of recovery plans in collaboration with the participants and assigned staff.
Act as a liaison, consulting with community agencies and families to maintain coordination of care in the treatment process.
Provides ongoing assessment of participants’ mental health symptoms and responds to recovery plans.
Ensures annual review of the participants physical condition and assist access to and encourage compliance with treatment for identified physical problems.
Board Certified Licensed LPN in the state of Delaware; or participating in the Nursing Licensure Compact to include the state of DE.
Knowledge of mental health systems and supports.
Successful completion of the Child Abuse Registry and the Adult Abuse Registry background check with no adverse registry findings, and obtain service letters in accordance with 19Del Code Section 708 and 11 Del Code Sections 8563 and 8564.
Local travel required, including transporting participants.
May be required to report to work during emergencies, including inclement weather.
May require crisis intervention in emergency situations.
