Job Description

ROLE - SPECIALTY

Assignment Details
Contract Length: 13 weeks
Guaranteed Hours: 40 per week
Requirements

• Current state license as a Registered Nurse
• Skills in Homecare Homebase Software
• OASIS experience
• CHPN certification (Preferred)
• Proof of current CPR certification
• Current driver's license and reliable transportation with insurance
• 1-2 years experience as a Registered Nurse in a clinical care setting
• 2+ years of home health experience

Roles & Responsibilities

• Assess and respond to the needs of patients and families in home health settings.
• Coordinate care with interdisciplinary teams, patients, and families.
• Implement, develop, document, and evaluate Plans of Care for patients.
• Educate patients and families on care, medications, diet, and safety practices.
• Supervise and provide clinical direction to other healthcare staff involved in patient care.
• Maintain accurate and up-to-date patient records and documentation.

• Additional duties and responsibilities may be assigned as needed.
