Job Details Homecare Coordinator of Care / CHHA RN will oversee the process of a patient’s recovery from illness, injury or surgery.
RN will do 5 to 6 patient visits per day (Monday thru Friday 8:30 am to 5 pm).
Implements and evaluates the home care needs of patients, including the assessment of a safe living environment, proper resources and support system of caregivers.
Communicates a plan of care for the patient between the physician, physical therapist, social worker and home health aide services.
Reconciles patient's medications with the Physician.
Proficient in placing wound vacs and Unna Boots when instructed by the physician.
Completes and submits OASIS assessments, reassessments, transfers, resumption of care, discharges, and significant changes in condition in accordance with Agency defined time frames.
Reviews patient insurance information to coordinate activities involved in each individual care plan.
Qualifications RN with active New York State Nursing license in good standing
Homecare / CHHA experience preferred
RN Must have a car / vehicle to be considered
