Senior Registered Nurse - Hospital at Home (Part-Time)

Work Schedule: Full‑time, in‑person in various senior settings with some opportunities to work from home part of the time.

Location: Must be comfortable with travel between Pocatello & Chubbuck, ID.

We are seeking a dedicated Registered Nurse (RN) to provide hands‑on clinical support for seniors in various settings, including individual homes, assisted living, independent living, memory care, and beyond. The RN will deliver direct patient care and extensive care coordination, working with residents, POAs, primary care providers, community groups, and on‑site senior living leadership to identify care gaps and implement chronic condition care plans to improve resident outcomes.

Conduct assessments, ongoing check‑ins, and other in‑person care for residents within senior living communities and other senior settings to identify and support care needs.

Assist Savoy’s Advanced Practice Providers in developing and implementing chronic condition care plans in collaboration with the clinical team.

Coordinate care for residents through collaboration with primary care providers, specialists, POAs/families, and other care services to ensure seamless patient care delivery.

Partner with an internal team, including a local Resident Care Coordinator, Partner Success leader, and virtual Advanced Practice Provider.

Maintain accurate and detailed documentation in Savoy Life’s Electronic Medical Record (EMR) and other technologies.

Triage after‑hours patient inbound phone calls to further support care.

Provide clinical expertise and act as a resource for on‑site staff to support high‑quality resident care.

Active Registered Nurse (RN) license in Idaho.

Strong background in geriatric/senior care, with a focus on care assessments and care coordination.

Experience in care planning for chronic conditions, including collaboration with interdisciplinary teams.

Excellent documentation and electronic medical record‑keeping skills.

Experience working in senior living facilities is highly desirable.

Strong clinical assessment and care planning skills.

Ability to work closely with both clinical and facility teams to coordinate care.

Adaptability in a new role and organization, with a focus on innovation and resident care.
