Position Title: Registered Nurse I
Contract Assignment Duration: 13 weeks
Schedule
Monday to Friday, 8 AM to 5 PM
Responsibilities
Visit patients in their homes to provide nursing care.
Conduct home health assessments, including Start of Care, Wound Care, and Follow-up visits.
Complete documentation and charting within 24 hours of patient appointments.
Maintain a weekly productivity expectation of 30 points, which typically involves seeing 5-10 patients per day.
Attend periodic office meetings as required.
Engage in on-call rotations and weekend duty as needed.

Travel Requirements
Candidates should be willing to travel up to 75 miles from the assigned clinic, ensuring reliable transportation is available.
Qualifications
Minimum of one year of recent experience in adult home health or hospice care.
Active nursing license required; pending licenses are not accepted.
Experience with HomeCare HomeBase (HCHB) and OASIS within the last six months is mandatory.
Certifications: CPR and BLS required.
General Expectations
Candidates will be responsible for driving to patients' homes daily.
Documentation must be completed accurately and promptly to ensure proper billing and patient care.
There are no guaranteed hours, and candidates need to meet productivity targets to fulfill assignment obligations.

Professional attire, including scrubs.
