Night Licensed Vocational Nurse (LVN) - Hospice

Under RN supervision, the LPN, Continuous Care is responsible for providing nursing interventions and teaching skills to maximize patient comfort and to enhance the quality of life to the patient/family unit.
A high school diploma or G.Current LPN licensure in the state of employment

• 6 months experience as LPN preferred
• Active BLS for healthcare professionals from the American Heart Association or Red Cross.
• For current Hope Employees: BLS should be obtained by January 1, 2025
• Mobile Driver - Valid driver's license and automobile insurance per Company policy
• If working in an inpatient unit certificate of completion of 30 hour Infusion Therapy Course and in-house 8-hour Infusion Class preferred
• Ability to communicate (written and verbal) in English
• Flexible schedule to meet the needs of patients
• Ability to use equipment with visual and auditory mechanisms
• Ability to perform the essential functions and physical requirements (including, but not limited to: lifting patients and/or equipment, bending, pushing/pulling, kneeling) of the job with or without reasonable accommodation
• Represent the Company professionally at all times through care delivered and/or services provided to all clients.

Under the direction of an RN, provides nursing tasks to patients and families such as continuous care, dressing changes, catheter changes as identified in the plan of care developed by the Interdisciplinary Group (IDG).
Demonstrates clinical skills regarding application of Company policies and procedures such as: medication administration, symptom management, and patient/family relationships.
Observes/monitors patients for alterations in body systems, pain and symptoms and reports to RN.
Observes and reports signs of adverse reactions and responses to treatments and reports to RN.
Reports findings to physicians, APRNs and physician's assistants following collaboration with RN.
Performs Central Venous Line (CVL) site observation/inspection; Documents per policy and procedure in medical record.
During times of emergencies (i.e. the LPN may be required to report to work at a location designated by the company, to ensure continuity of services. This may include reporting to work ahead of your scheduled date/time due to planned lock down of unit, and staying overnight(s) based on duration of emergency
Cross training to Hospice House positions of equal licensure/certification and scope when census calls for flexible staffing.
