Occupational Therapist (OT) - Home Health

Work Schedule: Monday-Friday (5 days/week)

As an Occupational Therapist (OT), you will play a vital role in helping patients regain independence and improve their quality of life in the comfort of their homes.

The Occupational Therapist provides skilled therapy services in patients' homes under physician orders. This role is responsible for evaluating patient needs, developing individualized treatment plans, implementing therapeutic interventions, and ensuring safe, effective, and compliant care delivery.
The OT also educates patients and caregivers, supervises Certified Occupational Therapy Assistants (COTAs), and ensures all documentation meets agency and regulatory requirements.
Clinical Care & Treatment Planning
Provide skilled occupational therapy services according to the physician's plan of care.
Observe, document, and report patient responses, changes, and outcomes to the physician and care team.
Patient Outcomes & Rehabilitation

Support recovery through progressive exercise and functional activity training.
Help reduce hospital readmissions through patient education and preventive care strategies.
Instruct on the safe and proper use of adaptive equipment such as walkers, wheelchairs, canes, braces, crutches, and prosthetic or orthotic devices.
Provide counseling and guidance throughout the rehabilitation process.
Supervision & Collaboration

Supervise and support COTA-provided patient care.
Collaborate with physicians, therapists, nurses, and other healthcare professionals to ensure coordinated care.
Complete accurate and timely clinical documentation in accordance with agency policies and Medicare/Federal guidelines.
Participate in in-service training and mandatory agency meetings.
Current Occupational Therapist (OT) license in the state of employment.
Minimum one (1) year of OT experience in an acute care setting.
Ability to meet the physical demands of home health care.
Previous home health experience.
