Activa Home Health is seeking a clinically strong Physical Therapist (PT) to support our Hospital at Home Program. In this role, you will deliver hospital-level physical therapy services in patients' homes, with a focus on mobility, balance, fall prevention, and discharge readiness.

Provide skilled physical therapy services to patients receiving hospital-level care at home
Perform comprehensive PT evaluations addressing:
Gait deviations and fall risk
Use of assistive devices and mobility equipment
Collaborate with physicians, nursing, OT, and care management to support safe transitions and reduce hospital readmissions
Complete timely and accurate documentation in compliance with Medicare, Hospital at Home, and agency standards
Active Physical Therapist (PT) license in the state of Florida
Degree from an accredited Physical Therapy program (DPT preferred)
