A skilled and compassionate physical therapist is needed to deliver home health services in a field-based setting. This contract assignment offers a rewarding opportunity to help patients enhance their mobility, strength, and independence within the comfort of their own homes.

13-week contract assignment

• Monday through Friday, 8:00 AM 5:00 PM
• Daily travel to patient homes (mileage reimbursement provided)
• Comprehensive electronic documentation using Home Care Home Base (HCHB)
• Utilize OASIS assessments for required care episodes

Complete thorough physical therapy evaluations, including OASIS assessments as needed

Deliver targeted interventions such as therapeutic exercise, neuromuscular re-education, transfer training, gait training, and fall prevention strategies

Monitor patient progress, update plans of care, and communicate changes with physicians and team members

Comply with agency procedures, Medicare guidelines, and home health standards

Active Physical Therapist (PT) license in Washington (in good standing)

Minimum of 1 year recent home health physical therapy experience

Proficiency with Home Care Home Base (HCHB) preferred

Reliable transportation, valid drivers license, and current auto insurance

$1,609 - $1,702/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
