LCSW- Mental Health Clinic
BMS Family Health and Wellness Centers
BMS Family Health Center is seeking a compassionate and dedicated Licensed Clinical Social Worker (LCSW) to join our behavioral health team. This role provides direct clinical services to adolescent and adult patients our primary care and school-based clinic settings.
The LCSW will deliver high-quality, patient-centered care through individual, family, and group therapy, while ensuring all documentation meets established mental health standards. This role also contributes to a collaborative, interdisciplinary care environment.
Fluency in English and Spanish is a plus!
Collaborate with the Director of Behavioral Health to support clinical goals related to adolescent and adult mental health services.
Conduct psycho-social assessments and patient needs analysis to develop treatment plans and appropriate referrals (e.g., psychiatric evaluation, hospitalization, in-home services).
Coordinate therapy sessions for patients and families.
Communicate and coordinate care with ACS, case managers, and other collateral contacts.
Maintain accurate and timely documentation in the Electronic Health Record (EHR), including closing progress notes within 48 hours.
Share relevant information with care partners to support optimal patient outcomes.
Masters degree in social work from an accredited program
- Valid New York State LCSW license
- 5 or more years of social work experience
- Bilingual in English and Spanish (advanced or native Spanish language speaker), preferred.
- Completion of Mandated Reporter Training
- Current CPR and Infection Control certifications
- Strong organizational, time management, and planning skills
- Strong knowledge of child and adolescent development.
- Ability to build positive, trusting relationships with youth and families.
- Excellent clinical documentation and professional writing skills.
- Proficiency with Microsoft Office Suite and Electronic Health Records.
- Ability to identify high-risk psychosocial factors affecting patient health.
- Participation in recredentialing and annual training requirements.
