RN MDS Coordinator - Skilled Nursing
$36.50-$49.00/hour | Part-Time (16 Hours per Week)

Competitive Pay: $36.50-$49.00 per hour (credit for experience)
Schedule: Part-time position working 16 hours per week

Growth Opportunities: Fast Hiring Process: Coordinate the Resident Assessment Instrument (RAI) process to ensure accurate and timely completion of resident assessments
Ensure MDS assessments accurately reflect each resident's physical, mental, and psychosocial status
Verify documentation supports services provided and reimbursement accuracy
Support interdisciplinary collaboration for accurate care planning, assessment completion, and ongoing case management coordination
Collaborate with clinical and administrative teams regarding admissions, transitions of care, and payer communication related to skilled nursing services
Monitor assessment schedules and ensure regulatory timelines are met
Assist with quality improvement initiatives and survey readiness efforts
Graduate of an accredited school of nursing with a current RN license in the state of employment
MDS 3.0 and case management experience in a skilled nursing or post-acute care setting required
Working knowledge of Microsoft Office applications
Previous long-term care or skilled nursing experience
