Licensed Practical Nurse (LPN)The Licensed Practical Nurse (LPN) functions as an experienced clinical team member, delivering skilled nursing care under the direction of a Registered Nurse (RN). This role requires sound clinical judgment, efficiency in patient care delivery, and the ability to supervise unlicensed staff while ensuring compliance with regulatory and facility standards.Core Responsibilities1. Clinical Assessment & JudgmentPerforms focused patient assessments and monitors clinical statusIdentifies, prioritizes, and reports significant changes in condition to the RNAnticipates patient care needs and contributes clinically relevant input to care decisions2. Care Planning & Outcome ManagementActively contributes to development, implementation, and revision of individualized care plansExecutes nursing interventions with minimal supervisionEvaluates patient response to treatment and escalates concerns appropriatelySupports strategies aimed at preventing avoidable re-hospitalizations3. Skilled Nursing CareAdministers medications and treatments safely and efficientlyPerforms skilled nursing procedures within scope of practiceEnsures timely, accurate clinical documentation in PointClickCare (PCC)Communicates pertinent clinical information to RNs, physicians, and interdisciplinary team membersAssists with provider rounds and clinical evaluations as needed4. Discharge & Transitional Care SupportParticipates in interdisciplinary care conferences as directedReinforces discharge planning, patient education, and care transitionsCommunicates care recommendations clearly to patients and families5. Leadership & Oversight of Unlicensed StaffDelegates and supervises patient care tasks in accordance with regulations and policyProvides real-time guidance and follow-upHolds unlicensed staff accountable for quality and completion of care6. Interdisciplinary CollaborationServes as a consistent clinical resource to nursing assistants and peersParticipates in effective shift-to-shift handoff communicationCoordinates care across departments to support timely and patient-centered outcomes7. Safety, Compliance & Professional PracticeMaintains medication security, including controlled substance countsAdheres to infection control, HIPAA, and safety standardsSupports quality assurance and performance improvement initiativesParticipates in mandatory education and assists with onboarding of new staffReports compliance or safety concerns per facility policy8. Operational ExpectationsDemonstrates flexibility to meet patient care and staffing demandsPerforms additional duties consistent with professional role and experienceRequired QualificationsGraduate of an accredited practical or vocational nursing programActive, unrestricted LPN license in the state of employmentDemonstrated experience in skilled nursing, rehabilitation, or geriatric careCurrent BLS/CPR certification (per facility/state requirements)Ability to communicate clearly and professionally in EnglishCompliance with TB screening and occupational health requirements
