Are you a dedicated Social Worker who believes compassionate care can change lives? At Frontier Hospice, we’re looking for a patient-centered professional to join our team.
As a Medical Social Worker (MSW) with us, your primary responsibility will be delivering exceptional social work services to our hospice patients and their families. You'll provide counseling and guidance to support their emotional well-being and coping capacity. Schedule: Monday - Friday, 8 am - 4:30 pm

Connect Hearts and Homes: Evaluate medical needs, home situations, financial resources, and community support.
Provide Individualized Care Advocacy: Implement personalized social work plans and collaborate with your Bereavement Coordinator.
Shape care plans, attend interdisciplinary meetings, and address psychosocial stresses.
Master of Social Work from an accredited school of social work
Bachelor of Social Work from an accredited school of social work (must be supervised by MSW)
Licensed as a Social Worker from the State Board of Social Work
Minimum one (1) year experience as a social worker in a hospice setting
Required: Minimum one (1) year experience as a social worker in a healthcare setting; long term care or medical surgical/acute care setting preferred
AL, AR, CA, GA, IL, MA, ME, NJ, NM, OK, OH, RI, SC, & TX: Current unencumbered license as a Social Worker from the State Board of Social Work
$33.00 - $35.00 per hour (Average Pay Range).
