Job Summary
Reports to the Operations Manager, the RN Nurse Navigator functions as a member of the multidisciplinary team of care providers who reflect a culture of cooperation, enthusiasm, and respect. The Navigator is responsible for coordinating the appropriate care of the patient based on an evaluation/assessment of the patient (telephonically or in person) that matches the patient’s desires to the most applicable program that the Surgery Department offers. This includes education, continuity of care and support through all phases of diagnosis, treatment, and follow‑up care for patients in order to maintain a high level of engagement and participation in the program that is critical to their success and the goals they desire. The role is mainly clinical, but also incorporates business development and a high level of patient engagement skills.

Job Type & Schedule
Regular, 40 hours per week, Monday through Friday, In Office. No nights, holidays, or weekends.

Licenses & Certifications

Valid driver’s license for travel between office sites.

Current registration to practice nursing in the state of Kentucky.

Obtains required CEUs, including all state required courses to maintain license.

BLS certification.

Active Kentucky RN licensure.

Education
Graduate from an accredited nursing program.

Years of Experience
3+ years of RN experience.

Duties and Responsibilities

Conduct patient assessments (telephonically or in person) to aid patients in navigating and selecting the appropriate Urology care pathway. Assist patients who are undecided in attending education/visit and information sessions regarding the program offerings to identify the program or provider that best meets their needs.

Provide a welcoming encounter (via phone or in person) for the patient, reflecting an ease of use into one of the Urology or Urogynecology locations and ease of getting timely access to education and information regarding the specialty’s offerings.

Assess patients clinically, use information gathered and protocols to make recommendations about the care pathway that best suits the patient’s needs.

Work with staff and support staff to guide patients through the chosen care pathway, ensuring they are informed and actively engaged. Perform minor outpatient clinical duties as directed by the physician within the scope of their licensure.

Coordinate with providers, outpatient departments, hospital departments, and other support staff to ensure patients are engaged, compliant, and well connected to the required services.

Collaborate with all Surgical Division programs to maintain patient preferences and address unique needs; may transition patients to other program offerings based on preferences and clinical information.

Participate in meetings and coordinate with various teams to improve patient care, satisfaction, and clinical outcomes that support departmental goals.

Remain current with research and treatment options.

Conduct follow‑up phone calls to assist patients with post‑discharge instructions, care continuity, results, and to reduce readmission risk. Seek patient feedback to improve the program.

Serve as an educational resource for staff; assist in developing multidisciplinary training programs.

Contribute to quality improvement plans and demonstrate willingness to participate.

Track patient outcomes, satisfaction, and financial data to support program processes and improvement activities. Report key statistics as necessary.

Required Skills and Knowledge

In‑depth knowledge of surgical and other treatment options for patients based on clinical assessment.

Knowledge of community resources and working proficiency with office equipment (computer, keyboard, printer, fax).

Excellent communication skills and computer literacy; proficient in EPIC EMR system.

Manage a clinic schedule, perform physical exams, interpret lab results, prescribe and manage medication, complete pre‑operative evaluations, and ensure patients meet insurance requirements for surgical services.

Navigate communication barriers between patients, families, disciplines, departments, and community agencies; promote collaboration.

Demonstrated ability to work successfully in a team‑based decision‑making culture.

Demonstrated ability to work independently, achieving effective outcomes and on‑time performance.

Plan and coordinate multidisciplinary communication strategies, strategic initiatives, and events.

Respond promptly and follow through on customer requests.
