Advanced Practice Provider Home Care

Are you passionate about providing compassionate care to patients in need? Join our team as a Palliative Care Advanced Practice Provider, where you'll have the opportunity to work directly with patients in their homes and various care facilities in the Ashland, Ohio area.

This field-based role allows you to deliver personalized palliative care, collaborating with a dedicated team to meet patients' medical and emotional needs. You’ll actively engage with patients and their families to develop customized care plans that align with their goals and overall health.

Your Impact:

Deliver palliative care services directly in patients' homes and other care settings.

Create and implement clinical care plans for adult patients facing complex health challenges.

Conduct thorough patient assessments, considering their goals of care and current medical conditions.

Engage with patients and families, providing education on medication, illness progression, and crisis management.

Work collaboratively with interdisciplinary healthcare teams to ensure seamless patient care.

Maintain accurate medical records and documentation in compliance with regulations and company policies using Epic EMR.

Stay committed to continuous education and professional development as required by state regulations.

Prescribe medications as authorized under state regulations.

Minimum Qualifications:

Master's degree in Nursing, MPAS, or MSPAS.

Current unrestricted Nurse Practitioner or Physician Assistant license in Ohio.

DEA registration or eligibility.

Current CPR certification.

Completion of a Tuberculosis test as required for the Carelon Health business unit.

Willingness to travel within the designated territory.

Preferred Skills:

Experience as an MPAS or MSPAS is highly preferred if a PA.

At least 2 years in home-based care, complex cases, hospice, or palliative care.

Experience in bedside care settings like hospitals, oncology, ER, or ICU is a plus.

Familiarity with Electronic Medical Records (EMR – Epic) is preferred.

Bilingual in English and Spanish is an advantage.

The salary range for this position is $107,680.00 to $134,600.00 .
