CenterWell Home Health, part of Humana Inc., is seeking a Home Health Physical Therapist to deliver one-on-one care in patients' homes across Wisconsin. This full-time role reports to the Clinical Manager and offers a sign-on bonus and relocation assistance.
You will plan and administer prescribed PT, assess strengths, develop rehab programs, supervise PTAs and aides, document progress, and coordinate with physicians and care teams to improve functional outcomes and quality of life.
