The Admission Nurse is a clinical professional responsible for initiating and resuming high ‑ quality home health services for patients referred to the agency. This role conducts comprehensive home assessments to determine skilled care needs, establishes an individualized plan of care in collaboration with the interdisciplinary team, and ensures timely, accurate documentation that meets regulatory, payer, and agency requirements. The Admission Nurse provides patient and caregiver education, evaluates home safety, and confirms eligibility for services. Through strong clinical judgment, excellent communication skills, and a patient ‑ centered approach, the Admission Nurse helps ensure a smooth transition into home care and sets the foundation for positive patient outcomes.

Create the initial comprehensive and interdisciplinary patient centered home health plan of care; and update the home health plan of care in collaboration with the physician or other authorized practitioner (AP)

Ensure agency compliance with regulatory and organizational requirements

Demonstrate expert knowledge unique to the practice of community health nursing

Provides high quality clinical care and teaching

• Create the initial comprehensive and interdisciplinary patient centered home health plan of care; and update the home health plan of care in collaboration with the physician or other authorized practitioner (AP).
• Ensure agency compliance with regulatory and organizational requirements.
• Demonstrate expert knowledge unique to the practice of community health nursing.
• Provides high quality clinical care and teaching.
• Associates Diploma in Nursing, required; Current RN License in NY (in good standing).
• Minimum of one year of clinical experience in a Certified Home Health Agency.
• Knowledge of Microsoft Office and experience documenting in a clinical software system preferred.
• Ability to work independently in a community setting.
