Weekend LPN Licensed Practical Nurse - Home Health

Position Overview Pride Health is seeking an experienced Registered Nurse (RN) to provide skilled home health services throughout Kanawha County. This role is responsible for conducting OASIS assessments, coordinating patient care, completing clinical documentation, and supporting patients in the home setting. The ideal candidate will have strong assessment skills, the ability to work independently, and a commitment to delivering high-quality patient care.

Patient Care Setting: Home Health
Schedule: Monday-Friday, 8:00 AM - 4:30 PM
Every third weekend and one additional day per week
Mileage Reimbursement: Yes, reimbursed between patient visits (first patient to last patient)

Complete OASIS Start of Care (SOC), Recertification, Resumption of Care (ROC), and Discharge assessments.
Perform comprehensive nursing assessments and develop individualized plans of care.
Document patient visits accurately and timely within the electronic medical record.
Monitor patient progress and communicate changes in condition to the interdisciplinary care team.
Coordinate care with physicians, therapists, caregivers, and other healthcare professionals.
Ensure compliance with Medicare, home health, and agency documentation requirements.
Participate in on-call rotation and provide after-hours support as required.

Current unrestricted Registered Nurse (RN) license.
Associate Degree in Nursing or higher.
Ability to travel throughout the assigned service area.
Ability to work independently in a home health environment.

Previous home health nursing experience.
Knowledge of Medicare home health regulations and documentation requirements.
Experience managing a patient caseload in a community-based setting.
